What is an enlarged prostate?

Benign prostatic hyperplasia, or BPH, is a non cancerous growth of the prostate, the gland wrapped around the urethra below the bladder. As it grows with age, it squeezes the urine channel, so the stream weakens and the bladder works harder.

Most men manage it with lifestyle changes or tablets; a procedure is needed when these stop working or complications appear.

Symptoms and warning signs

How an enlarged prostate shows itself

Symptoms matter more than size.

Common symptoms

  • A weak stream that stops and starts
  • Waiting or straining before urine begins
  • Waking twice or more at night to go
  • Sudden urgency and a bladder that never feels empty

When treatment cannot wait

  • Unable to pass urine. Acute retention needs a catheter the same day.
  • Repeated urinary infections or bladder stones
  • Kidney swelling or rising creatinine
  • Blood in the urine, which also needs a bladder cancer check
How BPH is diagnosed

Your assessment in one visit

Three tests guide every plan.

  1. 01

    Symptom score and PSA

IPSS grades symptoms; PSA screens for prostate cancer.

  2. 02

    Prostate ultrasound

Measures prostate size and any middle lobe to pick the procedure.

  3. 03

    Flow test and residual urine

Uroflowmetry measures the stream; a scan shows urine left behind.

A full range of treatments

Enlarged prostate treatment options

From tablets to laser surgery.

Labelled illustration of four enlarged prostate procedures: TURP removing the inner prostate with a wire loop, HoLEP laser lifting the adenoma off the capsule, Rezum steam injected into the prostate, and UroLift implants holding the lobes apart
How the main procedures open the channel: (1) TURP, (2) HoLEP, (3) Rezum, (4) UroLift.
Mild to moderate symptoms

Medication

Alpha blockers relax the prostate in days; 5 alpha reductase inhibitors shrink it over months.

Any size, most durable

HoLEP laser enucleation

A holmium laser lifts the obstructing tissue off its capsule, with no incision.

Up to 80 ml

Bipolar TURP

An electric loop removes the inner prostate in small pieces through the urethra.

30 to 80 ml, keeps ejaculation

Rezum water vapour

Steam destroys excess tissue, which the body absorbs over three months.

Under 80 ml, no middle lobe

UroLift implants

Implants pin the lobes apart like curtain ties; nothing is cut.

Large glands, unfit for surgery

Prostate artery embolisation

Tiny particles block the prostate's blood supply through a needle in the wrist or groin.

Which procedure fits your prostate

A treatment guide by prostate size

AUA and EAU guidance; your ultrasound decides.

Prostate volume Usual first choiceGuideline based If keeping ejaculation matters
Under 30 mlClose to normal size TURP or incision of the prostate UroLift
30 to 80 mlThe most common range HoLEP or bipolar TURP Rezum or UroLift
Over 80 mlLarge prostate HoLEP HoLEP, ejaculation sparing technique

A large middle lobe rules out UroLift. Check your score with the symptom checker below.

Frequently asked questions

What is the best treatment for an enlarged prostate?

It depends on your symptom score, prostate size and priorities. HoLEP works at any size; Rezum and UroLift keep ejaculation under 80 ml, per AUA guidance.

Can an enlarged prostate shrink without surgery?

Partly. Finasteride or dutasteride shrinks it by about a quarter over at least six months.

Does an enlarged prostate lead to prostate cancer?

No. BPH does not turn into cancer, but both can coexist, so every assessment includes a PSA test.

How much does enlarged prostate treatment cost in Turkey?

Typically 4,000 to 7,000 US dollars for HoLEP, including the hospital stay, against 12,000 to 20,000 dollars in the United States.

Will treatment affect my erections?

Rarely. Erections are usually unchanged; the main change after TURP or HoLEP is dry orgasm.
